Continuous improvement fails when observations are collected but never converted into owned actions.

Use one visible cycle: identify the issue, agree the expected change, assign an owner, implement the action and review the evidence on a set date.

The review is not the end. Its findings should alter the next plan, standard or control. That is what closes the loop.
